About this panel
When a gynaecologist, GP, or you yourself wants to look at the female hormonal axis end to end, this is the panel. Reviewed by a GMC-registered doctor with full written commentary. Two practical points to know before booking. (2) Hormonal contraception (combined pill, mini-pill, IUD, implant, injection) alters the natural hormone baseline; please tell us at booking so the clinician can document the contraceptive type and interpret your result accordingly. Timing of the draw also matters in cycling women: see the testing tip below. Progesterone confirms whether ovulation actually occurred, and prolactin is included because a raised prolactin is a treatable cause of cycle irregularity that is easily missed when nobody looks for it.

FSH (Follicle Stimulating Hormone)
Pituitary signal to the ovary; rises in perimenopause and menopause
LH (Luteinising Hormone)
Pituitary signal that triggers ovulation; may be raised in PCOS / PMOS though the LH:FSH ratio is not a Rotterdam diagnostic criterion (NICE CKS)
Oestradiol
The principal ovarian oestrogen; cycle-stage dependent
SHBG (Sex Hormone Binding Globulin)
Carrier protein - calculates the free testosterone fraction
Total testosterone
Total circulating androgen
Progesterone
Confirms whether ovulation occurred; read against the mid-luteal phase, roughly 7 days before the next period rather than a fixed day 21
Prolactin
Raised prolactin is a treatable cause of cycle irregularity and is easily missed; it rises with stress, recent exercise and even the draw itself, so it is interpreted with that in mind

Do I need to fast for the Female Hormone Panel?
No, fasting is not required for the Female Hormone Panel. You can eat and drink normally before your appointment. Not required for the hormones themselves. Timing matters far more than fasting on this panel: FSH, LH and oestradiol are read against days 2 to 5 of the cycle, and progesterone against the mid-luteal phase, roughly 7 days before your next period rather than a fixed day 21. Prolactin rises with stress, recent exercise and the draw itself, so an unhurried morning appointment gives the cleanest reading. We confirm the right day for you when the panel is designed.
